What Are Dental Implants?

A dental implant is a titanium device that is surgically placed in the jaw to replace the missing tooth or support artificial teeth. The titanium implant fuses with the jaw bone by a process called osseointegration and acts as a tooth root, which in turns helps in supporting the crown placed on top of the implant.

What is a dental crown?
A dental crown is a custom-made tooth-shaped cap that is placed over the damaged or decayed tooth to restore the shape, size, structure and function of the teeth. Usually, they are placed over the root canal treated tooth to restore the strength, on the implant root to restore the bite or in the front teeth to restore the aesthetics and a beautiful smile.

When are the Dental implants recommended?

  1.  Tooth loss due to decay or any other reason such as trauma or injury and there is no other way to save or restore the tooth.
  2. In case when there is a need to replace the missing teeth whether single or multiple.
  3. To act as a support for denture that is loose and there is a need for a better fit and function.

Components of a Dental Implant:

1. Implant Body/Titanium Screw: This part of the implant act as a tooth root and placed surgically in the jaw bone which then integrates with the bone.
2. Abutment: This part is screwed into the titanium post (implant). The design of the abutment depends on the type of the restoration that will be placed above the implant. It helps in connecting the implant with the Restoration.
3. Dental Restoration: It could be a single tooth crown, bridge or implant supported denture. Regardless of the type of the restoration, only the tooth or the bridge or the denture is visible in the mouth.

Types of Dental implant available in India

Main types of tooth Implants in terms of placement:

1. Endosteal Implants: Such implants are safe and the titanium screw of the implant is placed inside jaw bone.
2. Periosteal Implants: Such tooth implants are placed above the jaw bone and below the gum and is recommended in cases where the jaw bone is weak and is less defined.
3. Zygomatic Implants: Such tooth implants are less common and are used to correct defects in the upper jaw or to help restore the upper jaw that is edentulous or toothless.

4. Transosteal Implants: Such toot implants are recommended in the cases when there is severe bone resorption in the lower jaw. Hence, a plate is placed in the lower jaw with the help of the screws.

1. Single tooth implants: Single tooth implants are recommended in the cases where there is a need to replace a single missing tooth.
2. Implant supported bridges: Such bridge implants are recommended in the case where there is a need to replace multiple missing teeth in a row.
3. All-on-4 implants: Such tooth implants are recommended in cases where there is a need to restore the entire arch by placing 4 tooth implants in one jaw. The bridge that is placed over the 4 tooth implants can be removed for cleaning and can be screwed back. This option is a better alternative than traditional dentures.

4. 3-on-6 Implants: In this case there are three dental bridges are being supported by 6 dental implants in one jaw. It is a better alternative than traditional dentures.
5. Immediate Implants: Such implants are placed in the jaw bone immediately after the tooth extraction.

The Preparation before The Dental Implant placement Process

Full Dental Examination: When a patient arrives at our clinic, the Dental Implantologist will evaluate the oral cavity both clinically and radiographically with the help of an RVG X-ray (2D X-ray) to assess the available bone for dental implant placement. After analyzing the RVG X-ray, we may recommend a 3D X-ray called CBCT (Cone Beam Computed Tomography). CBCT provides detailed information about the bone structure, including bone height, density, quality, and underlying conditions such as osteoporosis or osteoarthritis.

Review of Medical History: Medical history of the patient is evaluated for conditions such as hypertension, diabetes, thyroid disorders, heart conditions, or blood disorders.
Treatment Planning: After analyzing the number of missing teeth and the bone condition of the target area, the Dental Implantologist will determine the number of dental implants and the type of tooth implants required to be placed in the area. After dental and medical examination and treatment planning, we will schedule a specific date and time for the dental implant surgery.

Steps of Dental Implant Placement :

Dental Implant surgery is a surgery that is done in stages with in between healing periods.

Local Anaesthesia and Dental Implant Placement: Dental implant surgery is performed under local anesthesia to numb the target area to ensure the procedure is painless. The dental implant specialist surgically places the titanium post/implant in the jaw bone by exposing the gum and drilling the underlying bone to create the space for placing the implant. In some cases, bone grafting may be required in cases when the bone is less dense. After placing the implant, the operated area is closed with the help of sutures (absorbable or non-absorbable). If non-absorbable sutures are used, the patient will be recalled after 7-10 days for suture removal.

Osseo-integration or Bone integration and Dental Implant Healing: It takes a minimum of 3 months for the dental implant to integrate with the bone(osseointegration). After 3 months, the patient is recalled to check the implant’s integration with the bone.
Once integration is complete, we proceed with the second stage of the dental implant surgery. In this phase, a healing cap is placed by exposing the gum over the implant. The intent of placing the healing cap is to protect the implant and prevent the growth of the tissue over the tooth implant.
Crown Placement: After 3 to 7 days, the patient is recalled for taking an impression or measurement of the implant to fabricate the crown. The impression is sent to the dental lab, where it typically takes 3 to 4 days to fabricate the crown. Once the crown is ready, the patient is called to check the fit. If the fit is correct, the crown is placed on the implant using a torque wrench to secure it properly.
Finally, the patient is scheduled for a follow-up appointment to ensure proper fit and comfort.

Post Dental implant placement care instructions –

  1. Dietary Restrictions: Soft-diet is recommended for first few days after dental implant placement.
  2. Maintenance of Oral Hygiene: Proper brushing and flossing should be done to avoid any food particle deposition around the treated area.
  3. Medication: Usually, the patient experiences mild pain and swelling in the treated area. Hence, one should take the prescribed medication regularly to avoid any infection or pain.
  4. Avoid Smoking: One should avoid smoking as it can delay the healing of the tissue and may cause implant failure.
  5. Follow-up: in case of any type of discomfort in fit or function, one must visit the dentist for follow-up.

The entire process from implant placement to crown fitting takes about 3-6 months in total. In case of Immediate dental implants, the procedure might take less time.
