Dentures

What Are Dentures?
Dentures are custom-made, artificial replacements that are designed to replace missing teeth and surrounding tissues and to restore chewing function, speech, and esthetics. There are two main types of Dentures—one is complete denture that replaces all missing teeth in both the arches and other is partial denture that replaces some missing teeth in one or both arches supported by remaining natural teeth.

Dental Crown And Dental Bridge

What is a dental crown?
A dental crown is a custom-made tooth-shaped cap that is placed over the damaged or decayed tooth to restore the shape, size, structure and function of the teeth. Usually, they are placed over the root canal treated tooth to restore the strength, on the implant root to restore the bite or in the front teeth to restore the aesthetics and a beautiful smile.
When Are Dentures Recommended?
Dentures are recommended in the following cases:
  • In cases when the patient has lost most or all the teeth due to age, decay, or gum disease.
  • In cases when the patient is looking for a cost-effective treatment plan to replace some or all the missing teeth. Have multiple damaged teeth that need extraction.
  • In cases when the patient wants to improve the chewing function, speech, and esthetics.
Types of Dentures Available in India
1. Complete Dentures:

Complete dentures are recommended when all the teeth in both the arches are missing.

• Conventional Dentures – These dentures are fabricated and placed after all the teeth are removed in both the arches and the supporting tissues have healed completely
• Immediate Dentures – These dentures are placed immediately after tooth extraction until the final dentures are made.
Pros: They help in restoring the function and smile and, are easy to remove and clean.
Cons: It can be uncomfortable for the patient initially to adjust to complete dentures and as they are usually made from acrylic, they can break on falling.

2. Partial Dentures:

They are recommended when there are some natural teeth left in the arch for support. They can be removable or fixed and can have metal clasps or metal framework for support.

• Cast Partial Dentures – These types of partial dentures use a cast-metal framework with acrylic resin prosthetic teeth to replace the missing teeth in the arch.
• Acrylic Partial Dentures – These types of partial dentures are fully acrylic (base and the prosthetic teeth) and are bulkier, less durable, and temporary options to replace the missing teeth.

• Flexible Partial Dentures – These types of dentures are made from flexible thermoplastic material such as nylon and have more advantages over traditional acrylic dentures. Flexible dentures are more comfortable in fit even during natural lower jaw movements, and esthetics and are more durable in nature (can last upto 8 years)
Pros: Removable partial dentures help in restoring function, fit and smile and is a cheaper option to implant supported dentures.
Cons: Removable partial dentures can be uncomfortable as they have a metal clasp that attaches to the natural tooth for support and this clasp can impinge on the gums of the supporting tooth. Also, they might cause harm to the natural tooth as they take support from them.

3. Implant-Supported Dentures:

These dentures are like traditional dentures and replace several teeth at once and instead of resting directly gums they use the support of dental implants to fix to the jaws. There can be removable and non-removable implant supported dentures depending on the treatment needs.
Pros: The implants supported dentures offer more retention and stability for chewing, and speaking. These implants are a great option for those who have a stable bone structure and who for those who find traditional dentures uncomfortable and difficult to adjust.
Cons: Costlier option as compared to traditional dentures, takes longer time and requires surgical procedure first to place the implants.

Benefits of Dentures
  • Dentures help in restoring the chewing function, speech and facial structure.
  • They also help in boosting self-esteem and social confidence. Boosts self-esteem and social confidence.
  • Dentures are a cost-effective option and are non-invasive compared to dental implants.
Step-by-Step Denture Making and Wearing Process

Step 1: Initial Consultation and Oral Evaluation
During the initial consultation, the dentist evaluates the oral condition by clinical and radiological examination. Xrays or scans may be needed to analyse the bone structure and left for support. The dentist also records the part dental and medical history and after thorough analysis, a suitable treatment plan is recommended.
Step 2: Impressions
After the treatment plan is decided, the patient is called for taking impressions of upper and lower arches that helps in creating a model of the mouth. These models guide for the fabrication of the dentures.

Step 3: Jaw Relation and Bite Registration
Jaw relation is the spatial relation that exists between upper and the lower jaw. Recording an accurate Jaw relation helps in the fabrication of the dentures that aligns with the natural lower jaw(mandibular) movements, enhancing comfort and functionality.
Step 4: Try-In
During this step, the mock -up dentures made from wax are tried in your mouth to check the fit, function and esthetics. Adjustments are made, if there are any misalignments in the dentures.
Step 5: Final Denture Fabrication
Dentures are fabricated after final adjustments are made. They are fabricated using high-quality acrylic or other durable materials.
Step 6: Fitting and Delivery
During this step, the final dentures received from the lab are tried on the jaws of the patient and are checked for the fit, bite and esthetics. Minor adjustments can be made if there is a minor misalignment. Guidance is provided for taking care of the dentures during this step.
Step 7: Follow-Up Appointments
Dentures might feel bulky initially and might cause soreness in the mouth (if not fitted properly). If there is still any discomfort with the new dentures, regular follow-ups are recommended to ensure comfort and function.

What Are the Drawbacks of Dentures?
  • Dentures are usually made from acrylic and have a particular thickness; hence one might feel that they are bulky and uncomfortable until one gets used to them.
  • Dentures might cause soreness or irritation in the gums initially or if the fit is not proper.
  • Dentures require proper cleaning and rinsing and storage instruction in denture solution or water in the night and should be taken care of properly as they have a chance to break after falling on the ground.
  • Might shift slightly while eating or speaking (unless implant-supported)
How Long Does It Take to Get Used to Dentures?

It usually takes 2–4 weeks to adjust to new dentures. It is common to experience minor soreness in the gums and extra production of saliva due to denture wearing. These symptoms reduce with time. One must practice speaking and eating soft foods while wearing dentures to adjust in transition.

How Long Do Dentures Last?

Dentures typically last for about 5-8 years. After that one may require new set of dentures or may require denture relining depending on the natural changes in the gums and bone structure.

Post-Denture Care Instructions
  • One must keep their dentures clean by brushing the dentures with soft toothbrush daily.
  • One must remove their dentures at night to give rest to the gums, and keep them soaked in water or denture solution in a denture box at night
  • Rinsing the dentures is necessary before wearing
  • One must schedule regular visit to their dentist in case of discomfort and for minor adjustments in the denture.
